Couples: two appointments back to back are possible, but not at the same time.
Come to the office with a completed intake form. We can snail mail this form The form is at the office, or here under "Intended Outcomes" on the main menu. One has to print this for, fill it in and bring to the appointment, or complete the form in the office ahead of the appointment. You need about 15 minutes to complete the task.
Gift Certificates: same fees as "Time on the Table " above.
Payment due: at the time of service (Cash, Local check, gift certificate, or credit card). There is a $35 service fee on returned checks. Check to see if Health Flex card covers massage. It is used like a credit card.
